ISO new page layout!

Looking to get a new page layout for my account. I want to use the same horse image that is at the top, that's not going to be changed. 

I need at least 4 text boxes. I'd like one that stretches the width of the page at the top, and then the other Boxes to perhaps be three that are side by side underneath this. 

I would like to use the same colour theme for this entire thing..
Offering up to $120,000 for it
